Care Network Cambridgeshire's free telephone support is available to clients and professionals Monday to Friday 9am to 5pm. Our Community Navigators help people to access local support and activities, including:
• Practical help around the home, including access to our free, short-term Help at Home service
• Support on hospital discharge to return home safely, and help to remain independent at home, via our Help at Home + service
• Support to manage well-being and build confidence, including referral to our Project Catalyst Wellbeing Services
• Information on Community Transport options
• Links to other local support services and enjoyable activities
